This code can only be entered one time during a mission. Any time during a mission press pause. While at the pause screen press UP, UP, DOWN, DOWN, LEFT, RIGHT, LEFT, RIGHT, L3, R3. You should hear a sound that you entered the code in correctly and your health and missiles will be refilled.

Ellen McNichol
You need to shoot down Albert every time he comes on the screen. This will unlock Ellen for the space missions.

Bonuses
Win the game to unlock cinemas, bonus planes, and levels.

Axelay
You must purchase all aircrafts for all pilots.

TwinBee
You must finish the game with a pilot from 2nd Element.

To Unlock Ellen (Ken's Ex). Go through the game and focus on Albert with ken. and shoot him down every time he is on the map. and evenutly you will see a cutscene of him finding Ellens past. Later on after that. there will be another cutscene of her landing at the base with a note from Albert.
NOTE - Ellen is only playable during the last few missions(Preferably The Space Ones)
